Re: Second opinion, it is time to rethink the opioid crisis, Globe, February 3, 2023
"As medical students in the late 1990s, we were told that if we prescribed opioids such as OxyContin which was introduced
in 1996 for "legitimate" pain, they were "safe." We need not worry about our patients becoming addicted.
When I became a staff emergency physician in 2001, it was perfectly normal to prescribe Percocet tablets to someone with a
twisted ankle, or a badly strained back. Somehow we believed that a magical divide existed between these "safe"
opioids that doctors prescribed for the "right" reasons, and those that people used for the "wrong' reasons."

This is a sobering and humbling statement from author Dr. Vincent Lam.
Historically opiate drugs as a class have been known for centuries to be addictive. Based on that experience medical practitioners
in 1996 would have raised a skeptical eyebrow or expressed frank derision at the claim by yet another drug manufacturer that
their latest about-to-be-introduced variation on the opiate theme was non addictive.
By that time medical practitioners were all thoroughly versed in the practice of evidence based medicine. However to my
knowledge no independent scientific article appeared in any of the five major Canadian, American and British general medical
journals of the day (CMAJ, NEJM, JAMA, Lancet, BMJ) which confirmed claims as to the long term safety of the prescription
of opiate drugs for any duration in the management of pain of any cause. On the contrary, many articles appeared which raised
concerns about long term effects.
Prior to 1996 use of the full spectrum of opiates was accepted as orthodoxy in the management of palliative care patients.
Similarly accepted as a practical matter was the prescription of short courses of specific opioids for the management of acute
pain of specific causes. However the issue of the overall safety of these drugs continued to be debated and remained ill defined,
encouraging the practitioner to be conservative in their use. This was accepted as a practical trade off between their known
benefits and their unknown risks. It was the 1996 promotion of opioids in chronic and acute non cancer pain which spread the
contagion described by Dr. Lam.
How did the subsequent about face in attitude on the part of practitioners leading to more liberal use of these drugs
occur? It seems that a lot of the promotion of Oxycontin came from representatives of the pharmaceutical industry, complicated
by the ever puzzling approval of the drug by the federal Canadian regulator, rather than from independent academic medical
scientists.
